Improving the quality of primary health care and follow-up for people with diabetes
ReflexD is an audit and feedback tool designed to transform practices, follow-up and care for people with diabetes.
A new way of thinking is necessary to improve the quality of health care.
The ReflexD team supports clinical teams and family medicine groups (FMG) in this process. We are collaborating with the Réseau de connaissances en services et soins de santé intégrés de première ligne (Réseau-1 Québec) and the Réseaux de recherche axée sur les pratiques de première ligne (RRAPPL).
Clinicians who want to engage in patient-centred health care use ReflexD as a reflexive analysis tool.
They learn from their patients’ trajectories, from evidence-based medicine and best practices in health care.
The Quebec Support Unit of the Strategy for Patient-Oriented Research is made possible under the Canadian Strategy for Patient-Oriented Research (SPOR) funded by the Canadian Institutes of Health Research (CIHR) and supported by the Ministère de la Santé et des Services sociaux du Québec and the Fonds de recherche du Québec—Santé (FRQS).
The ReflexD team seeks pioneer clinics, champion clinicians and partner organizations to scale these services
